• 1099 Hourly rate + Travel/Lodging + **MMA covered w/ TAIL**
• Schedule: Ideal is M-F; 8’s & 10’s (Cases usually run 700-2300)
*Will consider minimum of 2 weeks per month
• Call must be able to report within 30 minutes
• Setting: Hospital
• Cases: General cases, regional blocks, orthopedics, gynecology, and gastrointestinal
• Credentialing Timeframe: up to 30 days
• EMR Epic
• Active AZ license required upon presentation
