Facility – Sky Ridge Medical Center
Cross Credentialing Sites – Rose/PSL Medical Center, Parker Adventist Hospital, Castle Rock Adventist Hospital, and Swedish Medical Center
Credentialing Time Frame – 90 days
Anticipated Start Date – April
Length of Assignment – 6 months
Length of Shift – 8s, 10s, 12s
Call Requirement – Yes
· Call Schedule – In House & Beeper Call. 1-2 shifts per month
Types of Cases – All cases, spinals
EMR System – EPIC/Medaxion
CRNA’s Supervised – Yes
Work Week Guarantee – Yes
Additional Intake Notes – Must be open to call 1 – 2 x per month. In house and beeper.
Guarantee of scheduled hours.
Must choose 3 sites to be cross credentialed at to be considered.
